　　【摘要】 目的　為了探討硅膠假體隆鼻術(shù)的假體外露原因。方法　用兩種大小不同的假體置入兔的鼻背部，術(shù)前和術(shù)后分別測定白細胞數(shù)、淋巴細胞總數(shù) 和淋巴百分數(shù)，檢測IgE，并行組織的光學(xué)和電鏡檢查。結(jié)果　實驗組1(無 張力組)6只兔，均無假體外露，實驗組2(有張力組)6只兔中有4只發(fā)生假體外露，且有大量 細胞浸潤，炎癥反應(yīng)重，細胞器有病變。結(jié)論　假體外露主要原因是局部 張力過大，軟組織菲薄。

　　Experimental research on augmentation rhinoplasty with silicone Prosthesis

　　【Abstract】 Objective To research the cause of extrusion in augmentat ion rhinoplasty with silicone prosthesis.Methods　Two different sizes of silical prosthesis were implanted into the bridge of rabbits nose. D etection of white blood cell count,the total number of lymphcell and IgE,and exa mination of tissue by microscope and electromicroscope were made preoperatively and postoperatively.Results　None of the rabbits prosthesis wa s extruded in experimental group 1(no tension group),but in experimental group 2 (tension group) 4 of 6 rabbits prosthesis were extruded with a large quantity of cell infiltration, severe inflammatory reaction and the change in the cell u ltrastructure .Conclusion　The main causes of extrusion are loca l excessive tension and thinner soft tissue.

　　硅膠假體是近20年來較為流行的隆鼻術(shù)充填材料，但術(shù)后可能出現(xiàn)各種并發(fā)癥，其中 假體外露較為多見。原因多種多樣，不同的作者觀點不一：①除了手術(shù)者的技巧，如假體的 雕刻、埋置層次有關(guān)外，更主要的是與假體的形態(tài)為“L”形或柳葉形［1］有關(guān)；② 切口局部組織有慢性炎癥或瘢痕過多的組織［2］；③硅膠排斥及局部感染［3］ ；④柳葉形下端雕刻過尖，置入后因重力下移長期刺激致使鼻端皮膚破潰，或剝離的隧道 太淺［4］；⑤切口的縫合張力大［5］。

　　為此我們進行了動物實驗研究，以探討假體外露原因。
